Job Description
WeCare, LLC is hiring a Mental Health Practitioner to deliver Adult Rehabilitative Mental Health Services (ARMHS) to adults living with mental illness in community settings. This position focuses on rehabilitative, skill-building support that helps clients increase stability, improve daily functioning, and reach treatment plan goals. The practitioner works under the treatment supervision of a licensed Mental Health Professional and is expected to provide timely, compliant documentation and professional service integrity.
Responsibilities
- Provide ARMHS rehabilitative interventions (individual and/or group), focused on skill development and functional improvement.
- Deliver services that align with the client’s Individual Treatment Plan (ITP) and support measurable progress toward goals.
- Participate in assessment and planning activities, including gathering information and supporting the development and review of:
- Functional Assessment (FA)
- LOCUS/Level of Care activities and updates
- ITP updates, renewals, and periodic reviews
- Complete accurate, professional progress notes that support service integrity and audit readiness.
- Meet documentation deadlines consistently, including daily completion and timely signing of progress notes.
- Attend and actively participate in required supervision, case consultation, and documentation coaching.
- Coordinate professionally with internal administrative staff and external partners as needed for continuity of care.
- Maintain confidentiality, professional boundaries, and respectful communication with clients and team members.
Minimum Qualifications (Must Meet One of the Minnesota Pathways)
Applicants must have training working with adults and meet at least one Minnesota Mental Health Practitioner qualification pathway, including:
- Relevant behavioral science coursework and 2,000 hours of supervised experience providing services to adults with mental illness; or
- 4,000 hours of supervised experience in the delivery of services to adults with mental illness; or
- Graduate student internship/field placement in behavioral sciences or related fields; or
- Master’s degree in behavioral sciences or related fields; or
- Bachelor’s degree in behavioral sciences or related fields with a qualifying practicum/internship involving direct interaction with adults served.
Additional requirements:
- Ability to work in community settings and travel locally as required for services.
- Strong time management and ability to meet deadlines consistently.
- Ability to complete and sign progress notes on time, daily, and according to agency documentation standards.
- Basic to intermediate computer skills, including accurate data entry and use of electronic documentation systems.
- Strong professional writing skills and attention to detail.
Preferred Qualifications
- Prior ARMHS experience or community-based adult mental health experience.
- Experience supporting adults with complex needs, including housing instability, co-occurring conditions, or high service intensity.
- Bilingual ability and cultural competency that support client engagement.
Schedule and Work Setting
- Community-based service delivery (home and community locations) with coordination time for documentation and supervision.
- Supervision and team meetings may be in-person or virtual, depending on program needs.
